We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Coordinator you will oversee and enhance the delivery of our Aged Care services from the Ipswich to Lockyer Valley Region. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families. Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.You will provide a high level of service to ensure our clients and their families feel cared for and supported. You will predominantly work from home however it is required to travel to our client’s homes or to one of our offices, such as our Ipswich Office when required.
